To learn how to clip through fenestrated clip in paraclinoid aneurysm.
DesignThrough pterional approach and trans-sylvian dissection optico-carotid cistern was exposed and anterior clinoid was drilled intradurally, proximal dural ring was sharply cut and paraclinoid aneurysm was clipped by a fenestrated clip parallel to carotid.

InterventionDemonstration of pterional approach for clipping of paraclinoid aneurysm and its steps were intervened.
Avoidance of complications like intraoperative rupture, carotid artery injury etc were achieved successfully.
The steps of paraclinoid aneurysm clipping is well established. Prior anatomical concepts particularly drilling of anterior clinoid, proximal dural ring exposure and relationship of aneurysm to carotid artery is key to success.
